It's the "Diet and Excercise Assistant" v4.0 by Keyoe, Inc. (http://www.keyoe.com). Pretty nifty program actually. Has both a daily calorie and carb counter (with a decent pre-existing library) a daily logbook, and also helps track excercise and even Basal Metabolic Rate
